Kay Frances Formulak, of Anderson, passed away on August 13, 2026, at Ascension St. Vincent Anderson. She was born on May 12, 1935, in Oak Hill, West Virginia, to her parents, Effie and John Cobb.
Kay graduated from Collins High School in 1954, and earned an associate's degree from Concord College.
Following graduation, she moved to Washington D.C. and worked at the Library of Congress for Senator Kefauver. In D.C. she met her husband, Bill Formulak, whom she married on June 20, 1957, and together they had four children. They moved to Indianapolis, Indiana, in 1965 and ultimately settled in Anderson in 1974. Kay was a homemaker who cherished the time she spent caring for her husband and four children. She worked for Delco Remy after the children left for college.
Kay is survived by her three children, Brian (Lissa) Formulak, Mark (Laura "Jenny") Formulak and Pamela (Rick) Mackey; granddaughters, Lauren (Austin) Richardson, Christina (Kyle) Long and Kristin Formulak; grandson, John (Makenzie) Formulak; great-granddaughters, Nora Richardson, Brooke Formulak and Ellen Formulak and great-grandson, Weston Richardson; brother, Ronnie (Cathy) Cobb; sisters, Barbara Sargent and Karen Price.
She was preceded in death by her husband Wilfred "Bill" Formulak; daughter, Loriann Formulak; parents, Effie and John Cobb; sister, Betty Blizzard and brothers, Jimmy Cobb, Jack Cobb and Mike Cobb.
Visitation will be held on Wednesday, August 26, 2026, from 12:30 pm to 2:30 pm at Loose Funeral Homes & Crematory, 200 West 53rd Street Anderson, Indiana, 46013.
Services will follow visitation on Wednesday, at 2:30 pm at the funeral home with Rev. Mike Hopper officiating.
She will be laid to rest next to her husband at Anderson Memorial Park Cemetery.
